This program allows families to have a single point of contact that helps guide them through the process - reducing anxiety, saving time, and staying involved to ensure that each step goes as smoothly as possible.

Our collective personal and professional experience with the challenges of coordinating senior transitions has served as the basis for our development of a comprehensive service that supports our clients through the process from beginning to end:

• The process begins when an eldercare service or life transition need is identified
• It continues with a Benevia Transition Coordinator evaluating all of the potential needs of a client and developing a customized plan for their individual situation
• It progresses when one or more Benevia Provider Network™ services are engaged to address the clients specific needs
• Benevia stays involved to ensure that each step goes smoothly and helps in cases where challenges are met
• It ends when positive, successful outcomes have been achieved
